to offer sports physicals
The St. Mary’s Hospital Clinics have scheduled this years Sports Physicals. The Cottonwood Clinic will hold theirs on Wednesday, June 7 and the Nezperce clinic has scheduled their clinic on Monday, June 5th. The Kamiah Medical Clinic will be offering their annual program on Tuesday, June 13th. Each of those clinics will have special hours from 5:00 to 7:00 p.m. No appointments are necessary. 
“We can offer individual sports physicals at any time for the regular price, but we always set aside this particular time so we can offer them at the greatly reduced rate of $25 and return $15 to the schools for use in their athletic and academic programs,” said Jeanette Gorman, SMHC/CVHC Community Relations Coordinator. “The students will need the completed ‘Health Examination and Consent’ form signed by a health provider before they can begin practicing in the fall. Now is the time to have the physical completed, rather than waiting until the late summer or fall.” Students entering the 7th, 9th and 11th grade and all transfer students must have a completed sports physical form on file with the school prior to their team’s first practice. Students should bring the Health Examination and Consent form completed on the front side and signed by a parent or guardian. They should also bring their immunization records and a completed consent for immunization form signed by a parent or guardian. Both forms are being distributed to the students through their school. They are also available from the clinics. Immunizations will be provided at no charge during the scheduled Sports Physical clinics. A charge will be assessed if students have their physical at another time. Students should wear loose fitting T-shirts and shorts and wear their glasses or contacts. |
