St. Mary’s Clinics bring you the 20th annual Sports Physical Clinics for all area schools. This program provides the necessary sports physical screening required by the Idaho High School Activities Association. The cost of the exams are $25 (payable at the time of the exam) with $15 being donated back to the schools’ athletic department. No appointment is necessary. Students from any of the area schools are welcome to receive their physical at any one of the three clinics; Nezperce on May 18th, Cottonwood on May 19th or Kamiah on May 27th. Each clinic will be offering the sports physicals from 5:00 to 6:30PM. Students are asked to please wear shorts and a T-shirt or tank top to the exam. Also if you have prescription glasses or contacts please wear them for the physical. Immunization updates will be provided for no additional cost with consent; however students should bring their immunization records; as records will not be available during these sports physical clinics. The sports physical packets which include; a parent letter, flyer, health exam and consent form, immunization form and authorization for release of information form will be available at all area schools. Parents and/or guardians are asked to make sure they sign all the appropriate forms. If you did not get the Physical & Immunization forms from your school they may be picked up at one of the clinics in Cottonwood, Nezperce, Grangeville, Craigmont or Kamiah. Please note these physicals are only for Jr. & Sr. High School students so they can fulfill the Idaho High School Athletic Association requirements. |
