| Kindergarten screening dates set Kindergarten screening for the 2022-2023 school year will be held at the PRAIRIE ELEMENTARY SCHOOL (1916 East Street) on Tuesday, April 19th and Wednesday, April 20th This will take about 30-40 minutes for each child. Please bring your child’s state birth certificate. You can find the information and the form at: https://healthandwelfare.idaho.gov/services-programs/birth-marriage-death-records/about-birth-records Also bring your child’s current immunization record. State law requires each child to be fully immunized before the first day of school or sign an immunization exemption form at the screening.
